What a polygenic risk score can and can’t tell you

A polygenic score sums thousands of tiny genetic effects into one number — a direction, not a diagnosis. A plain-English guide to what a PRS means, what it doesn't, and why ancestry matters for how much to trust it.

A polygenic score distills your genetic risk for a condition into a single number. That makes it powerful — and easy to misread. It’s worth knowing what the number does and doesn’t mean before you read too much into it.

What a polygenic score actually is

For most common traits, no single gene is in charge. Thousands — sometimes millions — of common genetic variants each nudge your likelihood up or down by a tiny amount. A polygenic score (often called a PRS or PGS) adds all those tiny nudges together into one number. Think of it less like a switch and more like a very long, slightly weighted tally.

The score is relative. It places your genetics somewhere in a distribution compared to a reference group — roughly, whether your inherited loading for a trait leans lower, average, or higher than most people’s. It is a position on a curve, not a probability that something will happen to you.

What it can tell you

Used well, a polygenic score can add a signal that standard risk factors miss, because it’s fixed from birth and doesn’t wait for symptoms. For a handful of well-studied traits it can meaningfully refine the picture — a reason to keep up with the routine, evidence-based care your clinician already recommends for everyone, and to raise the result at your next visit. It’s a prompt for a conversation, best weighed alongside everything else your clinician already knows about you.

What it can’t tell you

  • It’s not a diagnosis. A higher score is not a condition, a prediction, or a timeline. Plenty of people with high scores never develop the trait, and plenty with low scores do.
  • Genes are one input.Environment, behavior, age, and plain chance often matter as much or more. A polygenic score shifts the odds; it doesn’t set them — and for many traits, the everyday, modifiable factors are the bigger lever.
  • Ancestry is a real limitation. The large studies most scores are built from have historically been overwhelmingly of European ancestry. Those scores transfer poorly to people of other ancestries and can be misleading for them. This is a real equity gap the field is actively researching but has not yet solved — and it belongs in the foreground, not the fine print.

How to think about your own score

Treat a polygenic score the way you’d treat a family history: useful context that can inform a conversation, never a verdict on its own. A higher-than-average score is a reason to ask your clinician whether the standard, evidence-based steps for that area make sense for you — not a reason to panic or to self-treat. A lower score doesn’t earn you a pass on the basics either. And if you’re of non-European ancestry, ask specifically how much weight the score deserves in your case.
